The Physical Examination is a necessary procedure to do once you’ve arrived in China, as it is required for the application of the Residence Permit.

Required Documents

• Passport (Original & Copy)
• Visa page with entry stamp (Original & Copy)
• 健康申请表 Health application form (Print out after making an appointment)
• 知情同意书 Informed consent form (Print out after making an appointment)
• Admission letter (Copy)
• 3x Passport photo (Available on site, paid)

Procedure

1. Book an appointment at https://online.shhg12360.cn/sithc/
   • Possible locations
     • 上海国际旅行卫生保健中心（浦东分中心）地址：上海市浦东新区金桥路2090号
     • 上海国际旅行卫生保健中心（金浜路分院）地址：上海市长宁区金浜路15号
2. Print out the appointment information
3. Go to the appointment
4. Return to the location on the day shown on your paper slip to retrieve the results
5. Pay in cash, Alipay, or WeChat pay
